Let’s talk single-needle access
When performing therapeutic plasma exchange (TPE) or red blood cell exchange (RBCX) procedures on Spectra Optia, you can choose the single-needle option. (Version 12 software is required for single-needle RBCX.)
This option offers a single point of access that provides intermittent flow rather than continuous flow and reduces the need for two peripheral needlesticks. This can improve the patient and operator experience, especially when dual-needle access is challenging.
Selection can be made at the beginning of the procedure or mid-run. To transition from dual- to single-needle access, select the single-needle option on Spectra Optia under the run menu button and options tab, then follow the steps on the screen. Once you’ve completed all the required steps, the single-needle icon will appear on the main run screen as confirmation.

Troubleshooting plasma collection in CMNC
When collecting plasma during a continuous mononuclear cell collection (CMNC) procedure, it’s important to remember that Spectra Optia prioritizes collecting cells over plasma when the end-of-run targets are configured to run time, whole blood processed, or total blood volume (TBV) processed.
This means that if you have programmed the system on the run values screen to collect plasma, and then you decrease the end-run targets (in the three configurations noted above) before plasma is collected, the plasma volume entered may change to zero.
When making changes, pay attention to the yellow arrows on the run values screen. If the plasma value has been zeroed, re-enter the desired amount to be collected. At the end of the procedure, on the run targets attained screen, verify you have the desired plasma before going into rinseback. If the plasma was not collected, re-enter the volume to collect, and the procedure will extend to collect the plasma.

Dear Access Point,
Do TPE and RBCX procedures require a central line?
— Mary in Las Cruces, NM
Dear Mary,
The short answer is no. Spectra Optia uses centrifugal separation, which does not require high inlet flow rates to maintain separation. Check out the “Peripheral Venous Access Tips and Tricks” in the Resource Spotlight section below to see what flow rates you can use with different IV catheter sizes. The flow rate range for exchange procedures on Spectra Optia is 5 to 142 mL per minute.

In each Access Point edition, we’ll feature an interesting fact, like this one.
The approximate total blood volume of an adult human is 5 L (10.6 pints). Compare that to the total blood volume of an adult African elephant — 245 L (65 gal).
